Fibromyalgia: It's the food, again! (probably).

Serendipity strikes again! On Facebook, I saw https://www.facebook.com/PaleoDietNewZealand/posts/763653980371516 . That linked to Fibromyalgia and non‑celiac gluten sensitivity: a description with remission of fibromyalgia.

For people unfamiliar with scientific terms, here are some definitions:-
Non-celiac gluten sensitivity: A reaction to gluten, not due to Celiac (Coeliac in the U.K.) Disease.
Remission: (medicine) An abatement or lessening of the manifestations of a disease.

So, is Gliadorphin-7 (formed during the digestion of gluten) to blame?
Probably.

Could Beta CasoMorphin-7 (formed during the digestion of A1 cow's milk) also be a problem?
Possibly.

Is Increased/Excessive Intestinal Permeability allowing the above large molecules to pass through insufficiently tight junctions?
Definitely, maybe.

Is there really no cure for Fibromyalgia? It's possible to tighten-up insufficiently tight junctions. Insufficiently tight junctions can be caused by:-

Wheat, Constipation, Ischaemic Heart Disease, Type 1 Diabetes, Schizophrenia and Autism.

"Prior to discussion it must be clarified that the hypothetical link between A1 consumption with autism spectrum disorder (ASD) and schizophrenia relates not to the cause of the condition but to the aggravation of symptoms associated with these neurological conditions. More specifically, the hypothesis states that the absorption of food-derived exomorphins such as beta casomorphin 7 (BCM 7) may aggravate symptoms associated with ASD or schizophrenia.

This hypothesis is the basis of 'dietary intervention' that excludes gluten and casein (Knivsberg et al., 2002) from the diet of ASD patients. The former, gluten, has been shown to release gliadamorphin, an exomorphin comparable in opioid activity to BCM-7. A number of laboratories in the United States and Europe offer urine tests, which determine the level of peptides including BCM 7 and other beta casomorphins to serve as an indication of the potential usefulness of dietary intervention in the treatment of ASD patients. One published study reports that a casein- and gluten-free diet was accompanied by improvement in 81% of autistic children within 3 months (Cade et al., 2000)."

"What is gliadorphin? Gliadorphin (also called alpha-gliadin or gluteomorphin) is a substance that resembles morphine. Ordinarily, this is a short-lived by-product from the digestion of gluten molecules (found in wheat, barley, rye, oats, and several other grains). Gliadorphin is very similar to casomorphin. Gliadorphin has been verified by mass spectrometry techniques to be present in unusual quantities in urine samples of children with autism, and are believed by many to be a central part of the system of causes and effects that cause autistic development. The most probable reasons for the presence of these molecules are:
* One or more errors in the breakdown (digestion) process caused by enzyme deficiency and/or
* Abnormal permeability of the gut wall (that would allow these relatively large molecules to enter the bloodstream from the intestine in abnormal quantities)."

Relating tissue HUFA balance with blood cholesterol and heart attacks. Results from the 25-year follow-up in the Seven Countries Study [35] were discussed in an earlier review [10] which noted that “Food energy imbalances which elevate blood cholesterol may be fatal only to the degree that omega-6 (n-6) exceeds omega-3 (n-3) in tissue HUFA. Such evidence raises questions about the hypothesis that blood cholesterol levels cause CHD.” Northern Europe and Southern Europe have abbreviations “No.” and “So.”, respectively. The Figure is reprinted with permission of the publisher.

Fig. 1 is interesting, as it shows a significant association between 25-year CHD mortality and Serum Total Cholesterol for every region except Japan. What's different about Japan, compared to Northern Europe, USA, Serbia, Southern Europe & Crete?

According to Measuring Blood Fatty Acids as a Surrogate Indicator for Coronary Heart Disease Risk in Population Studies , Philippines & Iceland have lower % linoleic acid than Japan. Where's the CHD vs TC data?

Could another difference be that the Japanese eat rice, a relatively intact grain, instead of foods made from wheat grain dust (i.e. flour) as their main source of dietary carbohydrates?

Things that make you go "Struth!"

I was wading through my Facebook News Feed when I spotted THIS. That article led me to New approach to coeliac testing identifies more Australians at risk, which in turn led me to A novel serogenetic approach determines the community prevalence of celiac disease and informs improved diagnostic pathways (provisional pdf), where I saw: "HLA-DQ2.5, DQ8, or DQ2.2 was present in 56% of all women and men in the community cohorts."
HLA-DQ2.5, DQ8 & DQ2.2 are the alleles for Coeliac/Celiac Disease (CD).

"Transglutaminase (TG)-2 IgA and composite TG2/deamidated gliadin peptide (DGP) IgA/IgG were abnormal in 4.6% and 5.6%, respectively, of the community women and 6.9% and 6.9%, respectively, of the community men, but in the screen-positive group, only 71% and 75%, respectively, of women and 65% and 63%, respectively, of men possessed HLADQ2.5, DQ8, or DQ2.2."
There were abnormalities in ~5% of Australian women & ~7% of Australian men, even in those who didn't carry CD alleles.

"...but based on relative risk for HLA-DQ2.5, DQ8, or DQ2.2 in all TG2 IgA or TG2/DGP IgA/IgG screen-positive subjects, CD affected 1.3% or 1.9%, respectively, of females and 1.3% or 1.2%, respectively, of men."
~1.6% of Australian women & ~1.3% of Australian men have CD.

From the discussion: "The concept of a ‘celiac iceberg’ has been important in drawing attention to a large, unrecognized group of patients with CD who do report symptoms considered ‘typical’ of CD [29]. Investigators have proposed expansion of the ‘iceberg’ to encompass patients who are genetically susceptible to CD, but show only raised IEL counts or an isolated abnormal CDspecific serology and normal intestinal histology [30-32]. Consequently, there is considerable uncertainty regarding the true extent of gluten-mediated disease in the community.

Random thoughts: About 1 in 20 Australian women & about 1 in 15 Australian men have some kind of a gut problem (IBS?) due to gliadin, even in those who don't carry CD alleles. The following made me smile.
"Making a diagnosis based on a blood test alone or commencing a gluten-free diet without a confirmatory bowel biopsy is inappropriate and can impose an unnecessary and lifelong treatment."
'Cos life without wheat, rye, barley & oats is such an imposition (undue burden) and everyone just loves to be given a bowel biopsy. <- sarcasm alert.

From Ancestry of Australian population: "More than 92 percent of all Australians descend from Europeans. Anglo-Celtic Australians (English, Scottish, Welsh, Cornish or Irish ancestral origin) make up 74 percent of the Australian population."
Most Australians have genes that originate from Britain & Europe. Uh-oh!

Why do only a small percentage of people carrying the CD allele go on to develop CD? I believe that it's down to luck. During digestion, gliadins are snipped into fragments & amino acids by the peptidase enzymes pepsin, trypsin & chymotrypsin. Gliadin fragments that contain the wrong triplet of amino acids and that manage to slip through excessively-loose tight junctions may trigger CD. Once the "damage is done", it only takes a tiny amount of gliadin to provoke an immune response.

"We identify the α-amylase/trypsin inhibitors (ATIs) CM3 and 0.19, pest resistance molecules in wheat, as strong activators of innate immune responses in monocytes, macrophages, and dendritic cells. ATIs engage the TLR4-MD2-CD14 complex and lead to up-regulation of maturation markers and elicit release of proinflammatory cytokines in cells from celiac and nonceliac patients and in celiac patients' biopsies. Mice deficient in TLR4 or TLR4 signaling are protected from intestinal and systemic immune responses upon oral challenge with ATIs. These findings define cereal ATIs as novel contributors to celiac disease. Moreover, ATIs may fuel inflammation and immune reactions in other intestinal and nonintestinal immune disorders."

"Among common food and feed protein products, soyabeans are the most concentrated source of trypsin inhibitors. The presence of high levels of dietary trypsin inhibitors from soyabeans, kidney beans or other grain legumes have been reported to cause substantial reductions in protein and amino acid digestibility (up to 50 %) and protein quality (up to 100 %) in rats and/or pigs."

"Normally encountered levels of phytates in cereals and legumes can reduce protein and amino acid digestibility by up to 10 %. D-amino acids and LAL formed during alkaline/heat treatment of lactalbumin, casein, soya protein or wheat protein are poorly digestible (less than 40 %), and their presence can reduce protein digestibility by up to 28 % in rats and pigs, and can cause a drastic reduction (100 %) in protein quality, as measured by rat growth methods. The adverse effects of antinutritional factors on protein digestibility and protein quality have been reported to be more pronounced in elderly rats (20-months old) compared to young (5-weeks old) rats, suggesting the use of old rats as a model for assessing the protein digestibility of products intended for the elderly."

I eat grains, also peas, beans & lentils, but not as a dietary staple. I make sure that they're thoroughly cooked at 100°C.

From Ataxia, peripheral neuropathy, and anti-gliadin antibody. Guilt by association?

"However, anti-gliadin antibodies lack disease specificity being found in 10% of healthy blood donors."

10% of the population that are healthy enough to donate blood have gut walls permeable enough to let fragments of gliadin pass through. Hmmm. If fragments of gliadin can pass through, maybe fragments of other hard-to-digest proteins can, too (e.g. casein).

I am NOT the anti-carb!

On a messageboard far, far away I was told:-

"While cakes and biscuits might be regarded primarily as treats, bread and pasta are staple foods, which form an important part of the diet of growing children, who are recommended by experts to get a fairly high proportion of their calories from carbohydrates, including wheat flour.

It is normal and healthy for children to have sandwiches and pasta as part of their meals, and pizza and pastry in moderation are perfectly appropriate. As are occasional treats.

I don’t suppose that I am the only person who is tired of your ceaseless evangelising on behalf of your own diet. But it is one thing to feed yourself as you please, and to talk about it if you wish, it is quite another to suggest that there is good reason for people to adopt your personal principles and ignore the advice of paediatricians, dieticians, and properly constituted advisory bodies when feeding children."

"I'm sorry Nigel, but you do give the impression of being on a one man crusade against wheat in our diets. You are also making the assumption that we all eat refined wheat. What about wholemeal bread, wholegrain cereals, wholemeal pasta? You are also assuming that we all buy sliced white bread. I make most of my bread in my bread machine, using organic flour. I know what goes into my bread, and it tastes so much better than supermarket bread.

Pasta need not be made from wheat, for example rice noodles and rice and corn pasta for coeliacs.

So many cuisines have wheat products in their diets. Obviously there are the different breads - nan, pitta, flatbread, tortilla wraps etc; a huge array of pastas, cous-cous; bulgar wheat. Wheat is also used in so many religious/celebratory foods - the bread at holy communion, Christmas/simnel/birthday cakes etc. I could go on."

In Europe and Northern American those staples are wheat and potatoes. Your argument that the eating of wheat is what makes westerners fat is entirely specious, as few of the more lean members of the culture have eschewed wheat, and your suggestion that the recommendations based on the food pyramid have failed at the population has got more obese since the pyramid was devised is a twisting of the facts – all the relaible evidence shows that the people who stick to the recommendations are just fine, it’s those that don’t follow the guidelines who become obese. The guidance in no way is a cause of the obesity epidemic.

In most of Asia the staple is rice (which, as eaten, is a refined grain incidentally, I don't know why you think it isn't)."



The above raises so many points that it's hard to know where to start. The beginning is probably the best place. WARNING! The following contains some irony.

"Experts" tell us what we should and shouldn't eat. That's worked so well, hasn't it? It's easy to blame people for not following the very guidelines that make them over-eat. It's their own stupid fault! "Experts" scoffed at Dr. Ignaz Semmelweis. He was more right than wrong though and modern medicine now uses sterile practices. "Experts" scoffed at James Lind and it took 42 years before the British navy adopted lemons or limes as standard issue at sea. So we know where "experts" can shove their dietary advice ... somewhere where the sun doesn't synthesise Vitamin D.

I don't try to get everybody on a low-carb diet. Jeez! Firstly, we are not all the same. Everyone has a different requirement for carbohydrate. As I wrote in Carbohydrates: Dogs' Doodads or Spawn of Satan?, "Right carbs, right amounts, right times." People don't do this, though. They eat the wrong carbs (powdered grains, mashed potato & other over-refined carbs like fruit juice) in the wrong amounts (way too much, as per "expert" advice) at the wrong times (virtually every meal, whether active or sedentary). Some random musings...

WHEATFLOUR: I guess it must be the lack of wheatflour that's pushed Orientals to the verge of extinction. Oh, wait...

LONGEVITY: Turn the clock back to a time before modern medicine and we wheatflour eaters had long average lifespans. Oh, wait...

BREAD: The vast majority of bread bought today is muck, mass-produced by the Chorleywood Bread Process. Even home-made bread is made from grain dust. See The problem with "Whole Grain" cereal etc. I don't consider white rice to be a refined grain as it still looks like a grain.

PASTA: When I was a lad, the only pasta we had was Heinz Spaghetti & Ravioli in tomato sauce. Now, pasta is a staple food in the UK? We all know how slim middle-aged Italians are. Oh, wait...

EXERCISE: When I was a lad, I used to run around in the street & playground like other kids. I was still fat. Exercise cannot compensate for poor diet (I ate lots of chocolate, cake & drank sugary Corona lemonade).

Why do we have to eat what farmers grow? Maybe if more people ate less powdered wheat products and more old-fashioned sourdough rye bread, farmers might start growing something else. There are plenty of carbohydrates that don't seriously disturb blood glucose levels, such as rice, rye, barley, quinoa, sorghum, millet, maize, sago, tubers, root veggies, bulbs, legumes & fresh whole fruits.

Nah, it'll never happen! Not while a) crap foods are dirt cheap and b) people keep following the dietary advice of "experts" like sheep ... or should that be lemmings?

The problem with "Wholegrain" cereals etc.

As I mentioned in Carbohydrates: Dogs' Doodads or Spawn of Satan?, some breakfast cereals turn into blood glucose faster than table sugar (half glucose bonded with half fructose) even though they're "Wholegrain" cereals.

A whole (i.e. intact) grain consists of a protective outer shell (a.k.a. bran) and innards consisting of starchy/proteiny endosperm and nutritious germ. See Cereal germ.

To turn a grain into a breakfast cereal, it's milled into dust, mixed with water to form a paste and the paste is extruded through holes into whatever shape the manufacturer desires. Technically, everything that was in the whole grain is in the finished product. However, the form and function have completely changed. Here are a couple of analogies.

1) I want to sell my car. I take it to a scrap-yard and have it shredded into tiny pieces. All of the tiny pieces are put in a skip, which is delivered to my driveway. I place a sign on the skip stating "Whole Mazda MX-5 for sale. Enquire within. £5,000 O.N.O.". What would you offer for my "Whole Car"?

2) An insane person gives me £5,000 (in £50 notes) for my "Whole Car". As I'm on a roll, I take the notes and put them through a cross-cut shredder which turns them into thousands of 2mm x 2mm pieces. I empty the pieces into an attaché case. Who will accept my attaché case full of "Whole £50 notes" as payment for their second-hand car? Any more insane people out there?

Roller-milling grains into fine dust does four things.

1) It exposes the starchy endosperm.

2) It greatly increases the surface area to volume ratio of the grain, resulting in faster digestion and absorption. Surface area to volume ratio is inversely proportional to particle size. If 3mm grains are roller-milled into 0.1mm particles, the surface area to volume ratio is 30 times bigger. See Particle size, satiety and the glycaemic response. Therefore, "wholegrain" breads made from roller-milled flours are as bad as white breads, in terms of glucose and insulin response. Choose wholemeal breads made from stone-ground flours, as the particle size is bigger.

3) It makes the finished product much more likely to stick to your teeth, resulting in the rapid formation of plaque that damages your teeth and gums.

4) It makes the finished product more energy-dense.

Rolled grains are grains that have been steamed (to cook and make them soft), then put through what's effectively a mangle. They're still intact, if somewhat flat. Puffed grains are grains that have been heated to make the water within boil. As steam takes up a much larger volume than water, the grains are inflated to a much larger size. They're still intact, if somewhat funky-looking!

Don't be conned by breakfast cereal labels. If they look like "O"s or Brillo pads or brake pads, they're not intact grains.

Oats are O.K. even when turned into oatmeal, probably due to their high beta-glucan content, which forms a wallpaper paste-like goo when wet. See Particle size of wheat, maize, and oat test meals: effects on plasma glucose and insulin responses and on the rate of starch digestion in vitro.

Gluten - more than just a pain in the guts?

Remember the advert "I'm feeling a bit bloated". "Here, have some Bifidus Digestivum!"? I wonder what percentage of the population suffers from bloating, gas pains, constipation, IBS or some degree of failure to absorb the nutrients from their food?

People with Coeliac Disease (CD) or Dermatitis Herpetiformis (DH) (intensely itchy spots on pressure points) have to avoid gluten as much as possible, as it produces an auto-immune response with antibodies that attack their own bodies. However, gluten is also implicated in other conditions due to molecular mimicry. Sjogren's syndrome (dry eyes & other bits) and cerebellar ataxia (brain damage) are mentioned in a huge article by Loren Cordain Cereal Grains: Humanity’s Double-Edged Sword.

This article suggests that there are conditions other than CD & DH which can be helped by switching from gluten-containing grains (wheat, rye, oats, barley & spelt) to non gluten-containing ones (rice, corn, quinoa, buckwheat, millet & amaranth). Luckily, supermarkets like Tesco, Waitrose and Sainsbury's now have a large "Free from" section, which makes finding gluten-free substitutes for breads, cakes, biscuits & breakfast cereals etc a lot easier.
